Pharmaceutical Care Manager

Care New England Health System seeks a skilled Clinical Pharmacist to optimize patient outcomes through evidence-based therapeutic guidelines. The successful candidate will thoroughly review and assess patients' medication regimens to identify, resolve, and prevent medication-related problems. They will apply their knowledge of pharmacotherapeutics, including oncology, nutrition, and biologicals, to make recommendations to providers and document all clinical activities accurately. A strong understanding of laboratory values, osmolarity, and other pertinent parameters is essential for this role.
Responsibilities
- Develop, implement, and maintain systems that promote desirable patient outcomes through the use of safe and cost-effective drug therapies
- Monitor drug therapies, interpret medication orders, and transcribe to patient medication profiles
- Provide accurate, adequate, and timely drug information to professional staff
- Collaborate with interdisciplinary teams to achieve desired patient outcomes
Salary
$130,000 - $160,000 per year
Requirements and Qualifications
A minimum of a Bachelor's Degree in Pharmacy from an ACPE-accredited school is required. A valid RI Pharmacist license is also necessary. Preferred qualifications include residency or 5 years of clinical experience, BLS/ACLS/PALS certification, and board certification in Pharmacotherapy.
