**About the Role**

Care New England Health System is seeking a skilled and dedicated Ambulatory Medication Therapist to join our team. As an integral member of our primary care offices, you will provide high-quality pharmaceutical care services, ensuring optimal medication regimens for our patients.

**Responsibilities**

  • Act as the medication expert on the interdisciplinary care team, providing evidence-based recommendations to achieve desired patient outcomes.
  • Reconcile and assess medication regimens for adherence and optimal therapy, promoting safe and effective use of medications.
  • Develop and implement quality improvement initiatives to enhance patient care, collaborating with providers and patients to ensure high-quality, cost-effective care.
  • Document all encounters within the electronic health record and communicate effectively with the interdisciplinary care team.
  • Contribute to achievement of contractual obligations for the ACO, including comprehensive medication reviews, adherence outreaches, and targeted medication conversion initiatives.

**Requirements**

To be successful in this role, you will need a Doctor of Pharmacy (PhD) degree or a Bachelor's degree in Pharmacy with 3 years of experience. Current RI Pharmacist license is required, and PGY-1 Residency or Board of Pharmaceutical Specialties certification is preferred. You must also obtain a Medication Therapy Management certificate within 6 months of hire.
