Learning Disability CHC Nurse Specialist

Job Summary
We are seeking a highly skilled and experienced Registered Learning Disability Nurse to join our team as a CHC Nurse Assessor. The successful candidate will be responsible for assessing and reviewing patients to determine their eligibility for NHS Continuing Healthcare (CHC) funding.
Main Responsibilities
The post holder will be responsible for:
- Conducting multi-disciplinary assessments and patient reviews in line with standard operating policy and procedures
- Supporting the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure that the workstream is planned and managed effectively
- Managing a clinical caseload, including resolving issues with funding, placements, invoices, and complaints
- Ensuring Continuing Healthcare guidance is followed and that services are delivered to maximise patient safety
- Supporting the development of personalisation across the organisation
- Providing accurate and legible recording of information
- Assisting/supporting individuals and representatives in relation to the NHS Continuing Healthcare process
- Operating within and providing enhancements to current management information, reporting to enhance decision making processes
- Contributing to the development of performance and governance strategies and the development and implementation of improvement programmes
- Delivering against objectives, performance targets, achieving quality outcomes, prioritising own workload, and working to tight deadlines
- Supporting training and induction of new staff
- Undertaking research and development as directed

Person Specification
The ideal candidate will have:
- Registered Learning Disability Nurse or other part of register with significant LD experience
- Evidence of post qualifying and continuing professional development
- Clear communicator with excellent writing and report writing skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences
- Ability to work on own initiative and organise own workload without supervision working to tight and often changing timescales
- Car driver / owner or reasonable alternative
