Job Summary

We are seeking a highly skilled and experienced Registered Learning Disability Nurse to join our team as a CHC Nurse Assessor for Learning Disability. The successful candidate will be responsible for assessing and reviewing patients to determine their eligibility for NHS Continuing Healthcare (CHC) funding.

Main Responsibilities

The post holder will be responsible for:

  • Conducting comprehensive assessments and reviews of patients to determine their eligibility for CHC funding
  • Developing and implementing care plans to ensure that patients receive safe and effective care
  • Working collaboratively with the multidisciplinary team to ensure that patients receive seamless care
  • Providing education and training to patients, carers, and healthcare professionals on CHC and related topics
  • Contributing to the development of policies and procedures related to CHC and learning disability care

Person Specification

The ideal candidate will have:

  • Registration as a Learning Disability Nurse with the NMC
  • Experience of working in a CHC or learning disability setting
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Knowledge of CHC and related legislation
