Senior Learning Disability Nurse Assessor

We are seeking a skilled and experienced Learning Disability Nurse Assessor to join our team at Coventry & Warwickshire Integrated Care Board. As a key member of our Learning Disability Team, you will play a vital role in ensuring quality of care and contributing to the culture of collaboration, creativity, and hard work.
As a Learning Disability Nurse Assessor, you will have the opportunity to use your assessment skills to support some of the most vulnerable individuals in the community. You will work autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review processes for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).
The ideal candidate will have knowledge of the National Framework for Continuing Healthcare (2022) and an understanding of the Mental Capacity Act 2005 and Mental Health Act 1983. You will be responsible for the day-to-day clinical and case management of a caseload of patients funded by NHS Continuing Healthcare (CHC), including those jointly funded with the Local Authority.
Key Responsibilities:
- Conduct assessments and reviews for patients with NHS Continuing Healthcare funding.
- Work with individuals and their representatives to support the assessment and review processes.
- Collaborate with the team to target specific elements of clinical review within the CHC service.
- Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure that the workstream is planned and managed effectively.
- Case manage a clinical caseload, including resolving issues with funding, placements, invoices, and complaints.
