Learning Disability CHC Nurse Specialist

We're seeking a motivated and experienced Learning Disability Nurse to work in a unique environment, supporting some of the most vulnerable individuals in the community.
This role is perfect for a candidate with ambition, who wants to make a difference in the lives of others. You will be working autonomously and collaboratively in a multi-agency environment with individuals and their representatives to support the assessment and review processes for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC).
The ideal candidate will have knowledge of the National Framework for Continuing Healthcare (2022) and an understanding of the Mental Capacity Act 2005 and Mental Health Act 1983. You will be responsible for the day-to-day clinical and case management of a caseload of patients funded by NHS Continuing Healthcare (CHC).

Main duties of the job
The post holder will be responsible for the assessment, review, and ongoing case management of patients funded by NHS Continuing Healthcare (CHC). This will include working as an autonomous practitioner, but also together with the team, targeting specific elements of clinical review within the CHC service for Coventry, Rugby, and Warwickshire.
The successful candidate will have the opportunity to contribute to the development of performance and governance strategies and the development and implementation of improvement programmes. You will be expected to deliver against objectives, performance targets, achieving quality outcomes, prioritising your own workload, and working to tight deadlines.
