About the Role

We are seeking a highly skilled and experienced Registered Learning Disability Nurse to join our team as a CHC Nurse Assessor. As a key member of our team, you will play a vital role in ensuring the quality of care for individuals with learning disabilities and their families.

Key Responsibilities

  • Conduct comprehensive assessments and reviews of patients funded by NHS Continuing Healthcare (CHC) to ensure that care is safe and effective.
  • Work collaboratively with the team to target specific elements of clinical review within the CHC service for Coventry, Rugby, and Warwickshire.
  • Support the Complex Case Manager/Clinical Commissioning Manager and the wider team to ensure that the workstream is planned and managed effectively.
  • Manage a clinical caseload, including resolving issues with funding, placements, invoices, and complaints.
  • Ensure Continuing Healthcare guidance is followed and that services are delivered to maximise patient safety.

Person Specification

  • Registered Learning Disability Nurse or other part of register with significant LD experience.
  • Evidence of post-qualifying and continuing professional development.
  • Clear communicator with excellent writing and report writing skills.
  • Ability to work on own initiative and organise own workload without supervision.
