As a valued member of our team, the Adult Community Mental Health Practitioner will play a pivotal role in delivering exceptional care to patients experiencing mental health-related challenges. Working within the Sexual Abuse and Assault Pathway, you will collaborate with multidisciplinary teams to develop and implement effective care plans, ensuring seamless transitions between services.

**Key Responsibilities:**

  • Manage a complex caseload, including specialist assessments, care planning, and liaison with relevant services.
  • Participate in quality improvement initiatives, contributing to audits, monitoring, and reviews of practice in line with current clinical guidance and policy.
  • Administer medication as appropriate, adhering to Trust procedures and NMC Guidance.
  • Develop, implement, and evaluate patient care programmes, promoting positive risk taking to enhance quality of life and independence.

**Requirements:**

  • Qualifications: RMN, DipSW, CQSW, CSS, or OT, registered with the relevant governing body (NMC, Social Work England, or HCPC).

**Estimated Salary:** £39,890 - £46,351 per annum, based on location and industry standards.
