About the Role

We are seeking a highly skilled and motivated Adult Social Care Specialist to join our team at Midlands Partnership NHS Foundation Trust.

This is an excellent opportunity for a qualified social worker to work with adults and older adults with mental health needs and their carers. As a key member of our team, you will be responsible for providing high-quality care and support to individuals, promoting independence and well-being.

Key Responsibilities:

  1. Conduct holistic assessments, care and support planning, and reviews to ensure individuals receive appropriate services.
  2. Work collaboratively with other Trust services to provide seamless care for adults and carers on defined pathways.
  3. Participate in duty/ACCESS or priority rota, screening/carrying out assessments for health and social care services using various assessment tools.

Salary and Benefits:

We offer a competitive salary of £32,000-£43,041 per annum, depending on experience, plus benefits such as a generous pension scheme, flexible working options, and extensive wellbeing support.
