Social Worker in Community Settings

We are seeking an experienced Social Worker in Community Settings to join our team at Midlands Partnership NHS Foundation Trust.
This role involves working with adults and older adults with mental health needs and their carers, providing high-quality care and support to promote independence and well-being.
Key Responsibilities:
- Carry out holistic assessments, care and support planning, and reviews to ensure individuals receive suitable services.
- Work collaboratively with other Trust services to deliver seamless care for adults and carers on defined pathways.
- Participate in duty/ACCESS or priority rota, screening/carrying out assessments for health and social care services using various assessment tools.
