Job Description

As a Community Support Worker - Adults with Midlands Partnership NHS Foundation Trust, you will play a vital role in supporting adults and older adults with mental health needs and their carers.

You will be responsible for carrying out holistic assessments, care and support planning, and reviews to ensure individuals receive the right services. You will also work closely with other Trust services to deliver seamless care on defined pathways.
