Supporting adults with complex needs in the NHS

We are looking for a skilled social worker to join our team in Newcastle, working with adults aged 18 and above who require support with physical and mental health needs. This role is a 12-month fixed contract position within our community-based Adult Social Care team.

As a member of our dynamic team, you will contribute to delivering high-quality public-facing services and participate in our Social Work Learning Academy, which offers Continuous Professional Development (CPD) aligned with the British Association of Social Work Professional Capabilities Framework.

Your key responsibilities will include:

  1. Undertaking assessments, care planning, and reviews to ensure the delivery of quality health and social care services.
  2. Collaborating with other Trust services to provide cohesive care pathways.
  3. Participating in the duty/ACCESS rota, utilizing assessment tools to screen and assess adults and carers.
