About the Role

This is an exciting opportunity to join a passionate and motivated multi-disciplinary team of professionals who provide expert care and support to individuals with dementia and their carers in the community.

Key Responsibilities
  1. Work collaboratively with other healthcare professionals to develop and implement comprehensive care plans for clients with dementia
  2. Provide expert advice and guidance to carers and families on managing dementia symptoms and accessing local resources
  3. Participate in regular team meetings and contribute to the development of best practice guidelines for dementia care
  4. Maintain accurate records of client interactions and care plans

Estimated Salary: £42,000 - £50,000 per annum (based on location and experience)
