About the Role:

We are seeking a highly skilled and compassionate Senior DIST Practitioner to join our team. As a member of our multi-disciplinary team, you will work closely with other professionals to provide assessment, support, and coordination of care for individuals with dementia and their families.

Main Responsibilities:
  • Manage and hold a caseload of clients with dementia and their carers.
  • Undertake initial assessments, including risk assessments, to identify client needs and develop personalized care plans.
  • Liaise with statutory and voluntary sector agencies to ensure timely access to care and support services.
  • Triage referrals and undertake duty role as required by the team.
