Dementia Support Practitioner

Norfolk and Suffolk NHS Foundation Trust provides mental health services, learning disability, wellbeing, older people's, and eating disorder services across Norfolk and Suffolk. Our team of practitioners care for approximately 23,000 people in the region, focusing on whole-life care that incorporates physical health, relationships, spirituality, culture, home, work, education, and purpose.
The Dementia Intensive Support Team is part of the Suffolk Care Group, working in the community to address the needs of individuals with dementia and their carers. As a senior practitioner, you will be responsible for:
- Undertaking initial assessments, including risk assessments, to identify service users' needs and develop tailored support plans
- Engaging with clients and their carers to provide personalized support and interventions
- Liaising with statutory and voluntary sector agencies to coordinate care and access necessary resources
