Job Description
This is an exciting opportunity for a Senior Practitioner to join our team. As a Senior Practitioner, you will:
- Work closely with other multi-disciplinary professionals to provide high-quality care and support to our clients
- Manage a caseload of clients with dementia and their carers
- Undertake initial assessments, including risk assessments, to identify client needs
- Liaise with statutory and voluntary sector agencies to get care and support in place as required
- Triage referrals and undertake duty role
- Attend Clinical and Business Meetings for the team
- Take on a supervisory role within the team
- Represent the team in meetings with partner agencies
What We Offer
We offer a competitive salary of £40,000-£60,000 per annum, depending on experience, plus a range of benefits, including flexible working hours, up to 307 hours holiday entitlement, and FREE physio service.
