A Career Opportunity in Dementia Care
We are seeking a highly skilled Senior Practitioner for Dementia Services to join our multi-disciplinary team. As a key member of our Dementia Intensive Support Team, you will work with people with dementia and their carers to maintain independence and quality of life.
The ideal candidate will possess excellent communication skills, be able to work collaboratively with others, and have a strong understanding of dementia care principles.
Your primary responsibilities will include:
- Managing and reviewing client caseloads
- Completing comprehensive assessments and developing individualised care plans
- Providing expert advice and guidance to clients, families, and other healthcare professionals
- Participating in multidisciplinary team meetings and contributing to the development of care strategies
- Developing and maintaining effective relationships with external partners and stakeholders
