Dementia Intensive Support Team - Senior DIST Practitioner
This is an exciting opportunity to join a passionate and motivated team of professionals who support people with dementia and their carers to remain living independently in the community.
The team's philosophy centres on early intervention and specialises in working with those who are hard to engage with. The team aims to prevent unnecessary hospital admissions and premature moves to long-term care.
As a Senior DIST Practitioner, you will work closely with other multi-disciplinary professionals within the team to:
- Manage and hold a caseload
- Conduct initial assessments, including risk evaluations
- Engage and collaborate with clients with dementia and their carers
- Liaise with statutory and voluntary sector organisations to arrange necessary care and support
- Triage referrals and undertake duty responsibilities
- Participate in clinical and business meetings for the team
- Assume a supervisory role within the team
- Represent the team in meetings with partner agencies
