Dementia Care Coordinator

We are excited to offer a challenging and rewarding role as a Dementia Care Coordinator within our Suffolk Care Group. As part of our Dementia Intensive Support Team, you will work with clients and their carers to provide high-quality care and support in the community.
Key Responsibilities- Develop and implement personalized care plans for clients with dementia
- Work collaboratively with other professionals to ensure seamless care delivery
- Maintain accurate records and reports on client progress
- Nursing degree or equivalent qualification
- Experience working within a multidisciplinary team
- Familiarity with mental health legislation and policies
- Competitive salary £43,141 - £72,679 per annum
- Flexible working hours
- Up to 20 days annual leave
