Patient Care Coordinator

We are seeking a highly motivated and organized individual to join our team as a Patient Care Coordinator. In this role, you will be responsible for coordinating care across the ward/department, deploying staff and prioritizing as necessary.

- Work closely with the Ward Manager to ensure the smooth operation of the ward.
- Develop and implement plans to improve patient outcomes and satisfaction.
The successful candidate will have excellent communication and organizational skills, and be able to work effectively in a fast-paced environment. They will also have a strong background in nursing, with experience of coordinating care and developing plans to improve patient outcomes. This is a full-time position, with a salary of approximately £40,000 - £55,000 per annum, depending on experience.
