Care Coordinator for Women's Mental Health

About the Role
We are seeking a dedicated and compassionate Care Coordinator to join our team at The Chimneys Clinic, a specialist care service designed to support women with emerging or personality disorders.
As a Care Coordinator, you will play a vital role in supporting our multidisciplinary team to deliver high-quality care that promotes recovery, personal growth, and empowerment. Your day-to-day activities will include ward rounds, analysing patient behaviours, and adapting care plans accordingly.
You will also be responsible for updating patient records, providing overall care and support, which may include assisting with personal hygiene tasks. A positive attitude, compassion, resilience, and vigilance are essential qualities for this role.
The shift pattern is 12.5 hours per day, three to four days a week, including nights and weekends. You will work closely with colleagues to create activities that promote mindful practice, engagement, and cognitive-behavioural change.

The Chimneys Clinic is a 12-bedded service that provides specialist care specifically designed to support women with emerging or personality disorders. Our focus is on enabling young women to develop effective strategies to manage harmful behaviours and achieve their personal life goals.
