Job Description

This exciting opportunity allows you to join a passionate and motivated team of professionals supporting individuals with dementia and their carers to maintain independence in the community. As a Senior Dementia Intensive Support Team Practitioner, you will manage caseloads, conduct initial assessments, engage with clients, and liaise with statutory and voluntary sectors to provide necessary care and support.

You will also attend clinical and business meetings, take on supervisory roles, and represent the team in partner agency meetings. With flexible working hours, up to 307 hours annual leave, and free physio services, this role offers an excellent work-life balance and opportunities for professional growth.
