About the Role

We are seeking a highly skilled Clinical Care Coordinator to join our team at South West London and St George's Mental Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for coordinating care plans for individuals experiencing first episode psychosis.

Key Responsibilities

  • Assess health and social care needs and negotiate care plans with service users and their carers
  • Work closely with other members of the multidisciplinary team to ensure delivery of care plans
  • Engage service users and carers assertively and offer care/interventions within the least restrictive setting
  • Provide a service sensitive to age, culture, ethnicity, gender, sexuality, and disability

Requirements

  • RMN qualification
  • Experience of care coordination and working with individuals with severe mental health problems
  • Knowledge of Mental Health Law and NHS and Community Care Act 1990
