Mental Health Care Coordinator

We are seeking a highly skilled and experienced Community Psychiatric Nurse to join our Kingston and Richmond Early Intervention Service (EIS) as a Care Coordinator.
The successful candidate will work within a multi-disciplinary team to provide intensive care to individuals experiencing first episode psychosis, with the aim of preventing relapse, reducing hospital admissions, and maximizing personal and social functioning.
Key Responsibilities- Coordinate care plans for individuals with first episode psychosis, working closely with other team members and external agencies.
- Assess and manage crisis situations, using evidence-based interventions and risk management approaches.
- Provide a range of psycho-social interventions, including assertive community treatment and relapse prevention planning.
- Work collaboratively with service users, carers, and other professionals to achieve positive outcomes.
- Participate in the development of early detection strategies and public awareness campaigns.
We are looking for a candidate with:
- Registered Mental Health Nurse (RMN) qualification.
- Post-qualification training or education in mental health.
- Experience of care coordination and working with individuals with serious mental health problems.
- Knowledge of mental health law and NHS and Community Care Act 1990.
- Excellent communication and interpersonal skills.
