Clinical Assessment Practitioner

About the Role
We are seeking a highly skilled and experienced Clinical Assessment Practitioner to join our team at South West London and St George's Mental Health NHS Trust. As a Clinical Assessment Practitioner, you will be responsible for assessing and managing patients with severe mental health problems, working closely with other healthcare professionals to provide high-quality care and support.
Key Responsibilities
- Conduct thorough baseline mental health assessments and case formulation based on these
- Assess and manage crisis with those with a severe mental illness
- Provide thorough written assessments reports/discharge letters for all allocated clients
- Carry out Duty functions on a Rota basis
- Enter information onto Rio in a timely manner in line with Trust guidelines and prepare reports as appropriate
- Implement and contribute to policy and service development as appropriate
Person Specification
- Essential:
- Social worker or occupational therapist with current relevant registration
- Practice Assessor qualification
- Desirable:
- Diploma/BSC in OT or Social Work
- Evidence of post registration training related to the provision of relevant psychosocial interventions
