About the Role

We are seeking a highly motivated and experienced Clinical Assessment Specialist to join our team at South West London and St George's Mental Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for assessing and managing referrals, providing high-quality care to patients, and working closely with other healthcare professionals to ensure seamless service delivery.

Key Responsibilities
  • Assess and manage referrals, identifying mental health needs and developing effective care plans.
  • Provide high-quality care to patients, including triage, assessment, and treatment planning.
  • Collaborate with other healthcare professionals to ensure effective service delivery and patient outcomes.
  • Contribute to the development of policies and procedures, ensuring they are evidence-based and aligned with best practice.
  • Participate in clinical meetings and contribute to the development of the team.
Requirements
  • Registered social worker or occupational therapist with current relevant registration.
  • Practice Assessor qualification.
  • Excellent written and oral communication skills.
  • Skills in assessment, care planning, and case management.
  • Ability to assess clinical situations quickly and use initiative.
  • Knowledge of counselling skills, cognitive skills, and behavioral skills.
Desirable Qualifications and Experience
  • Diploma/BSC in OT or Social Work.
  • Evidence of post-registration training related to the provision of relevant psychosocial interventions.
  • Skills in CBT, Coaching, and family work.
  • Research and audit skills.
  • Report writing skills.
  • Demonstrable experience of working with people who experience serious ongoing mental health problems in a variety of settings.
  • Experience of working in a multidisciplinary team.
  • Experience of assessing and managing crisis with those with a severe mental illness.
