About the Role

We are seeking a highly motivated and experienced Clinical Assessment Practitioner to join our team at South West London and St George's Mental Health NHS Trust. As a key member of our multidisciplinary team, you will be responsible for assessing and managing referrals, providing high-quality care to patients, and working closely with other professionals to ensure seamless service delivery.

Key Responsibilities
  • Assess and manage referrals, identifying mental health needs and developing effective care plans
  • Provide high-quality care to patients, including triage, assessment, and support
  • Collaborate with other professionals to ensure effective service delivery and patient outcomes
  • Contribute to the development of policies and procedures, ensuring best practice and compliance with regulatory requirements
  • Participate in clinical meetings and contribute to the development of the team
Person Specification
  • Essential:
    • Social worker or occupational therapist with current relevant registration
    • Practice Assessor qualification
  • Desirable:
    • Diploma/BSC in OT or Social Work
    • Evidence of post-registration training related to the provision of relevant psychosocial interventions
