We are seeking a highly skilled and dedicated Clinical Assessment Practitioner to join our experienced multi-professional service team at South West London and St George's Mental Health NHS Trust.
Job SummaryThe successful candidate will be responsible for assessing and managing referrals from GPs, external providers, and self-referrers, and will work closely with a multidisciplinary team to determine the appropriate treatment pathway for patients.
Main Responsibilities- Assess and manage referrals from GPs, external providers, and self-referrers
- Work closely with a multidisciplinary team to determine the appropriate treatment pathway for patients
- Provide high-quality care for patients referred to the service
- Support patients through triage and assessment review
- Liaise with other disciplines and organisations to ensure seamless care delivery
