About the Role

We are seeking a highly skilled and dedicated Community Psychiatric Nurse to join our Early Intervention Service at South West London and St George's Mental Health NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care to individuals experiencing first-episode psychosis.

Key Responsibilities
  • Coordinate care plans for individuals in the early stages of psychotic illness, aged 18-65.
  • Work closely with other team members to assess health and social care needs and negotiate care plans with service users and their carers.
  • Undertake risk assessments and ensure delivery of care plans in partnership with multi-disciplinary colleagues and health and social care agencies.
  • Engage service users and carers assertively and offer care/interventions in the least restrictive setting.
  • Provide a service sensitive to age, culture, ethnicity, gender, sexuality, and disability.
  • Offer evidence-based psycho-social interventions and proactive risk management approaches to help service users and their families reduce and manage symptoms, high-risk behavior, and other disabling effects of psychosis.
Person Specification
  • Essential:
    • RMN qualification
    • Evidence of post-qualification training/education
  • Desirable:
    • Formal training in psychosocial interventions or equivalent
Requirements
  • Experience of care coordination
  • Demonstrable experience of working with people who experience serious ongoing mental health problems
  • Experience of assessing and managing crisis in people with severe mental health problems
Skills and Abilities
  • Knowledge of Mental Health Law & NHS and Community Care Act 1990
  • Knowledge of our business and how it supports patient care
  • Ability to maintain and enhance effective working relationships both internally and externally
  • Ability to plan, allocate, and evaluate own work and develop self to enhance performance
