Clinical Assessment Specialist

About the Role
We are seeking a skilled and dedicated Clinical Assessment Practitioner to join our experienced multi-professional service team at South West London and St George's Mental Health NHS Trust. The successful candidate will be responsible for assessing and managing caseloads of patients referred to our service, providing high-quality care and support to individuals with severe mental health illnesses.
Main Responsibilities
The successful candidate will be responsible for:
- Assessing and managing caseloads of patients referred to our service
- Providing high-quality care and support to individuals with severe mental health illnesses
- Working closely with other disciplines and organisations to ensure seamless care delivery
- Providing written assessments reports and discharge letters for allocated clients
- Carrying out duty functions on a rota basis
- Providing information and reports as required, and accurately documenting all relevant information regarding clients and staff
Requirements
To be successful in this role, you will need:
- Qualifications in social work or occupational therapy, with current relevant registration
- Practice Assessor qualification
- Excellent written and oral communication skills
- Skills in assessment, care planning, and case management
- Ability to assess clinical situations quickly and use initiative
- Knowledge of counselling skills, cognitive skills, and behavioral skills
