Adult Care Coordinator

- Offer support and supervision to your team of Social Workers and Senior Practitioners.
- Lead, coach, and support them to deliver high-quality support to our clients.
- Undertake assessments, support planning, review, and case management to support adults with learning disabilities and/or autism.
To be successful in this role, you will need a professional social work qualification (DipSW, CQSW, CSS) or other social work qualification certified by Social Work England.
You will also require:
- Motivation by the purpose of your role – to help and support others.
- A proven track record of a person-centered, and asset-focused outlook with high professional standards.
- Excellent supervision skills and an ability to enable practitioners to develop and thrive in their roles.
