Job Title: Care Coordinator Role

We are seeking a dedicated and compassionate care coordinator to join our Out of Hours Community Nursing team. As a care coordinator, you will provide high-quality patient care in patients' own homes, working on your own initiative and as part of a team.

Main Responsibilities:
  • Co-ordinate care for patients with complex needs.
  • Develop and implement care plans for patients.
  • Work closely with other healthcare professionals to ensure seamless care delivery.
About the Position:

Requirements:
  • Current NMC registered general nurse (degree in nursing).
  • Relevant experience in caring for patients with complex needs.
  • Evidence of clinical knowledge relevant to the post.
