As a member of our team, you will play a vital role in delivering patient-centered care in diverse settings, such as private homes and residential facilities, as well as clinics. Your core responsibilities will involve:

  • Evaluating patient needs and developing tailored care plans
  • Managing complex medical cases, including those requiring palliative care
  • Collaborating with multidisciplinary teams to coordinate care
  • Delivering high-quality wound care and management services
  • Supporting patients in preventing hospital readmissions

To excel in this position, you must possess:

  • NMC registration as a qualified general nurse (with a degree in nursing)
  • A valid UK driver's license and dependable transportation
  • Substantial experience in community-based healthcare settings, preferably in similar roles
  • Strong knowledge of care planning principles and best practices in Information Governance
