Midlands Partnership NHS Foundation Trust is seeking a dedicated and compassionate nurse to join our Out of Hours Community Nursing team in South Staffordshire.

We are offering a competitive salary package, estimated at around £30,000-£40,000 per annum, depending on experience. In addition to the salary, we offer excellent benefits, including flexible working patterns, generous annual leave, and opportunities for career development and progression.

As a Band 5 Nurse with our team, you will be responsible for delivering high-quality patient care in patients' own homes or residential homes, as well as providing care in clinic settings where appropriate. You will assess care needs, develop and implement care plans, and evaluate their effectiveness.

Key responsibilities include:

  • Prioritizing treatment and care for individuals according to their health status and needs
  • Providing palliative and end-of-life symptom management
  • Catheter management and procedures
  • Wound management
  • Long-term condition management
  • Preventing hospital admissions

To succeed in this role, you will need:

  • Current NMC registration as a general nurse (degree in nursing)
  • A valid UK driving licence and access to a car
  • Relevant experience in healthcare, preferably in community nursing
  • Evidence of clinical knowledge relevant to the post, including care planning and Information Governance
