We are seeking a highly skilled Registered Nurse to join our Community Health Team as a Community Health Nurse Specialist. This role involves providing high-quality, patient-centered care to individuals in their own homes.

About the Role

This position is ideal for a motivated and compassionate nurse who enjoys working independently and as part of a multidisciplinary team. You will be responsible for assessing patients' health needs, developing personalized care plans, and delivering evidence-based interventions to promote optimal health outcomes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical and emotional health needs
  • Develop and implement individualized care plans that address patients' unique requirements
  • Deliver high-quality nursing care to patients in their own homes, including wound management, medication administration, and education on healthy living practices
  • Collaborate with other healthcare professionals, such as GPs, specialists, and social workers, to ensure seamless care coordination
Requirements

To be successful in this role, you must possess:

  • A valid NMC registration as a Registered Nurse (Adult)
  • Experience working in community nursing or a similar setting
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues
  • A strong understanding of evidence-based practice and its application in community nursing
Working Conditions

This role requires flexibility, as you will need to work across various locations, including patients' homes, GP surgeries, and community centers. A full driving license and access to a car are essential, as you will need to travel to different areas during the working day.

The salary for this position is estimated to be around £38,000 per annum, based on the title, job description, and location.
