Community Health Specialist

Birmingham Community Healthcare NHS Foundation Trust is seeking a skilled Community Nurse to join our team in providing high-quality care to children in the community. As a Community Nurse, you will work closely with clients, schools, and other professionals to deliver evidence-based care and support children with medical conditions.
The role involves completing health care plans, delivering core medical needs training to school staff, and working in partnership with clients and other professionals to evaluate the care given and share outcomes. You will also carry out home visits and work in wider community settings.
We are looking for a motivated and compassionate individual with excellent communication skills, who is passionate about delivering high-quality care and making a positive impact on the lives of children and their families.
Main Responsibilities:
- Complete health care plans for children with medical conditions
- Deliver core medical needs training to school staff
- Work in partnership with clients and other professionals to deliver evidence-based care
- Carry out home visits and work in wider community settings
