Community Public Health Nurse

About the Role
Birmingham Community Healthcare NHS Foundation Trust is seeking a highly skilled and enthusiastic Community Public Health Nurse to join our team. As a key member of our Health Visiting and Early Years team, you will play a vital role in delivering high-quality services to children and families in Birmingham.
Key Responsibilities
- Undertake health needs assessments and screening for children, providing prompt feedback and escalation of concerns as required.
- Participate in the planning, delivery, and evaluation of health promotion activities, utilizing evidence-based practice.
- Contribute to the BFS early help offer, supporting families to access additional support as required in line with the early help framework.
- Contribute to the safeguarding of children at risk, in line with appropriate Safeguarding policies and procedures.
- Participate in and promote childhood immunization programs as appropriate.
- Participate in the Trust staff nurse development program and other related Trust-based learning opportunities.
- Work within the NMC code of professional conduct.
- Support Health Visiting Teams in the management of children with additional medical needs, completion of Educational & Health care plans, and preparing/supporting families for transition to education.
- Support and deliver evidence-based packages of care for children, in line with the BFS Standard Operational Procedures for Health Visiting and clinical pathways.
- Liaise with other Multi-Disciplinary Teams to ensure effective communication across the teams to support the HV oversight and co-ordination of care.
- Support the Health Visiting and Early Years teams to deliver Early Help intervention and complete Early Help Plans as required.
- Keep accurate and contemporaneous electronic patient records for children and families, in line with NMC requirements and the Trust's Record Keeping policies and guidelines.
- Convey information, of a complex nature, to other professionals and to clients, with well-developed oral communication skills.
