Role Overview
As a Community Nurse, you will be responsible for delivering high-quality care to patients in their own homes, GP premises, and health centres. You will assess and evaluate patients' health needs, develop and implement individualised care plans, and work closely with other healthcare professionals to achieve optimal patient outcomes.
Responsibilities
Your key responsibilities will include:
- Conducting assessments and evaluations of patients' health needs.
- Developing and implementing individualised care plans.
- Providing education and support to patients and their families.
- Maintaining accurate records and communicating effectively with patients and healthcare professionals.
Requirements
To succeed in this role, you must have:
- A current NMC registration.
- Excellent communication and interpersonal skills.
- The ability to work autonomously and as part of a team.
- A full driving licence and access to a car.
Salary and Benefits
We offer a competitive salary of £25,600 - £32,000 per annum, depending on experience, plus excellent benefits, including:
