Main Responsibilities

The Complex Discharge Team at University Hospital Southampton NHS Foundation Trust (UHS) is seeking a highly organized and detail-oriented individual to support safe and timely patient discharges.

  • Collaborate with the multidisciplinary team (MDT) to coordinate patient discharges, ensuring seamless transitions to community services.
  • Maintain accurate patient discharge records in the complex discharge database (APEX) and patient notes.

  • Basic knowledge of medical terminology.
  • Proficiency in word processing/typing (RSA 2 or equivalent experience).
  • Experience in medical or patient services administrative roles.
