Care Coordination Role

This is an exciting opportunity to work as a Care Coordinator in our Norwich office. The successful candidate will be responsible for coordinating the care needs of service users, working closely with care workers, other professionals, and internal teams.

Responsibilities
  1. Coordinate the care needs of service users, working closely with care workers, other professionals, and internal teams.
  2. Provide information and support to service users and care workers.
  3. Liaise with external agencies and professionals to ensure seamless care delivery.
  4. Assist in recruiting and onboarding new care workers, ensuring they have the necessary skills and training.
  5. Conduct regular supervisions and appraisals of care workers.
  6. Support the effective handling of safeguarding incidents, complaints, and concerns.
  7. Ensure compliance with CQC standards and regulations.
Essential Requirements
  • A valid UK driving license is essential for this role.
  • Flexible and able to adapt to changing demands and priorities, remaining calm under pressure.
  • A friendly, confident, and helpful telephone manner is crucial for this position.
