About Our Role

We are seeking a skilled Specialist Social Care Practitioner to join our Home First Team at Falkirk Community Hospital. This is an exciting opportunity for a qualified Social Worker or Occupational Therapist to work with adults across all Forth Valley hospital sites and intermediate care settings.

Key Responsibilities

As a Specialist Social Care Practitioner, you will be responsible for assessment and care planning in hospital discharges. You will also have experience of working with statutory legislation and guidance relating to adult services social work, as well as managing complex casework and assessing and managing risk.

You will have the ability to chair meetings and support team practice development through mentoring, facilitating student placements, groups, or communities of practice. Requirements

You should be qualified for a minimum of three years and have experience of working in a busy and dynamic hospital discharge team. You will be required to gain/maintain PVG scheme membership.
