Carr Gomm is a leading provider of social care services, and we are seeking a highly skilled Support Practitioner to join our team. The estimated salary for this role is £35,000 - £40,000 per annum.

Job Description

We are looking for a dedicated individual to support people with complex needs, including social support, medication management, personal care, and practical assistance. The successful candidate will have excellent communication skills, empathy, and the ability to work independently.

Responsibilities:

  • Providing daily living support, including assisting with household tasks, managing medication, and promoting overall wellbeing
  • Developing and implementing personalized care plans to meet the unique needs of service users
  • Maintaining accurate records and reporting any changes or concerns to relevant staff members

Requirements:

  • Experience working in social care, preferably with individuals with complex needs
  • Excellent communication and interpersonal skills
  • Able to work flexible hours, including evenings and weekends
  • Hold a relevant qualification, such as SVQ or equivalent
