Health and Social Care Advocate

As a Health and Social Care Advocate at Potters Health Care, you will be part of a dynamic team working towards improving the lives of our clients. This role comes with a competitive salary of £24,000 - £30,000 per annum, depending on qualifications and experience.
You will be responsible for supporting clients in various aspects of their lives, including community access, shopping, and social activities. Light house duties and carer respite experience are desirable, and school hours are available for those seeking a better work-life balance.
We encourage applicants with a background in social care, but training is available for the right candidate. A SVQ2, HNC in Health and Social Care, or equivalent qualification is necessary. Strong English communication skills, a driving licence, and car ownership are also essential. Proof of right to work in the UK is a must.
Key Responsibilities:- Support clients' physical, emotional, and social needs, prioritizing their well-being and safety.
- Monitor and report any changes in clients' conditions, advocating for their rights and promoting adherence to health and social care standards.
- Maintain accurate records of client interactions, ensuring evidence-based practice that respects dignity, respect, choice, compassion, wellbeing, and responsiveness to changing needs.
- Collaborate with clients, family, friends, and agencies to ensure seamless care and support.
