About the Role:

We are seeking a skilled and experienced Health Visitor to join our team in the Wiltshire region. As a key member of our community health team, you will play a vital role in delivering high-quality patient care and improving health outcomes for individuals and communities.

Main Responsibilities:

  • Deliver comprehensive health visiting services to families and children in the Wiltshire region.
  • Conduct home visits and assessments to identify health needs and provide personalized support.
  • Develop and implement effective care plans to promote healthy lifestyles and prevent illness.

Requirements:

  • A recognized qualification in health visiting (SCPHN).
  • Valid UK driving license and access to a vehicle.
  • A passion for delivering high-quality, person-centered care.
  • A drive to continuously develop professionally and expand your skill set.
