We are looking for an experienced Advanced Clinical Practitioner to join our multidisciplinary team and contribute to the delivery of safe, effective, well-led, responsive, and quality patient care. The successful candidate will be responsible for providing high-quality patient care and working closely with our team to maintain patients in a community setting and avoid hospital admissions and facilitate discharges.

Key Responsibilities
  • Contribute to the development and implementation of policies and procedures within their sphere of responsibility
  • Practice autonomously across all four pillars of advanced practice
  • Work closely with medical and GP colleagues to maintain patients in a community setting
Requirements
  • MSC Advanced Practice or equivalent level 7 education relative to role
  • Independent prescribing
  • Extensive experience of working as ACP band 7
  • Experience of managing and leading teams
