Senior Support Coordinator

We are seeking a dynamic and passionate individual to join our team as a Support Coordinator. In this role, you will have the opportunity to work closely with our team to deliver high-quality support to individuals with learning disabilities.
The successful candidate will have a strong commitment to person-centred planning and will be able to effectively communicate with our team, families, and external care professionals.
Key Responsibilities:
- Lead a team of frontline staff to deliver high-quality support and achieve personal outcomes for individuals
- Develop and deliver robust service designs, support strategies, and risk assessments
- Manage individual budgets and ensure effective use of resources
- Work closely with the team to create and implement person-centred plans
- Communicate effectively with families and external care professionals
Requirements:
- Experience working with adults and children with learning disabilities, epilepsy, autism, and physical support needs
- Strong facilitation skills to encourage staff teams to take ownership and responsibility
- Excellent organisational skills and ability to prioritise workload
- SVQ Level 3 in Health and Social Care or equivalent
- Full driving licence with access to own vehicle for business use
