**Job Title:** Clinical Support Worker for Patient Rehabilitation
**Location:** Buckinghamshire, UK
We are seeking a motivated and skilled Clinical Support Worker to join our team in Buckinghamshire. As a Clinical Support Worker, you will play a vital role in providing support to patients who have recently been discharged from hospital, helping them to regain their independence and remain in their own homes.
The ideal candidate will be able to take a holistic and person-centred approach to supporting patients to recover from their recent hospital admission and to achieve their short-term goals, maximising independence. The role will include visiting patients in their home, providing therapeutic treatment of patients under the guidance and supervision of qualified therapists within the team.
