Complex Neuro Rehabilitation Specialist

The role of the Adult Case Manager at Buckinghamshire Healthcare NHS Trust is a challenging and rewarding opportunity for an experienced professional to drive improvement, efficiency, and transformation in planning, commissioning, and delivering specialized services.
Main Responsibilities- To support patient flow from acute care through to specialist rehabilitation and community care by using data-led understanding of patient needs.
- To work with providers to identify patients requiring specialist rehabilitation and ensure appropriate use of rehab beds.
This will involve resolving 'bottle-necks' in specific units across the localised ICB and Southeast region, as well as identifying barriers, reporting on unmet need, and highlighting areas of development.
As part of the Community and Rehabilitation Care Group at the National Spinal Injuries Centre, this role will be accountable to the Specialised Commissioning team, providing a clinical interface to commissioned rehabilitation services.
