About the Job

We are seeking a skilled and compassionate Clinical Reablement Worker to join our Home First Therapy team in Buckinghamshire. As a key member of our team, you will play a vital role in supporting patients who have recently been discharged from hospital, enabling them to regain their independence and live comfortably in their own homes.

Responsibilities include:

  • Visiting patients in their home to assess their needs and deliver therapeutic treatments.
  • Working closely with senior Therapy staff to ensure seamless delivery of care.
  • Completing and delegating non-clinical tasks within the Home First therapy team.
Requirements
  • NVQ level II or III in Health Care.
  • Commit to undertaking Foundation Degree and any associated training once available.
  • Manual Handling or First Aid qualification.
