Vulnerable Adults Care Coordinator

We are looking for a highly motivated Vulnerable Adults Care Coordinator to join our team in Cornwall. As a key member of our community, you will be working closely with vulnerable adults who require support and guidance.
Job Description- Provide one-to-one support to customers living in the community, ensuring their safety and wellbeing.
- Work collaboratively with specialist providers to deliver tailored support plans, helping customers achieve greater independence and access local services.
- Develop and maintain relationships with referral partners, including local Mental Health Teams and Adult Social Care in Cornwall Council.
- Conduct regular risk assessments to ensure the service remains suitable for customers.
The ideal candidate will have excellent communication skills, be able to work under pressure, and possess accurate record-keeping abilities.
What We Offer- A competitive salary of £14.45 per hour.
- An essential car user allowance will be provided, as driving between customers is a large part of the role.
- A full-time contract with the possibility of extension.
